Once you are pre registered a member of our office staff will be in contact with you about further instructions
On Day of First Appointment
Arrive 20 minutes prior to your scheduled appointment to complete the check-in process
Bring to every visit:
- Photo ID
- Insurance Card(s)
- All medications in their original bottles
- Co-Payment
- A translator, if necessary
Insurance Information
Healthcare Institute of North Florida accepts most PPO insurance plans. Please contact our office for any questions.
We do not accept any HMO insurance.
