NEW PATIENT INFORMATION
Prior to First Appointment
Preregister with our practice using one of the methods below:
- Select patient, then patient portal link at the top of the page and follow the prompts
- Print out the documents required and either mail them or drop them off at the office
- Stop by our office and pick up the required forms and either fill them out there, mail them, or drop them back off at the office
On Day of First Appointment
Arrive 20 minutes prior to your scheduled appointment to complete the check-in process
Bring to every visit:
- Photo ID
- Insurance Card(s)
- All medications in their original bottles
- Co-Payment
- A translator, if necessary
Insurance Information
Healthcare Institute of North Florida accepts most PPO insurance plans. Please contact our office for any questions.
We do not accept any HMO insurance.
